Requesting a repeat prescription

If you need a repeat prescription for medication previously prescribed or managed by our clinician, call +31 6 2066 5465 or email info@fa-mc.nl. State the medicine name, strength, dose and any relevant details clearly.

Your request will be reviewed before a prescription is issued. We will contact you if a consultation or additional information is needed and send confirmation when the request has been processed.

Do not stop, start or change medication without discussing it with an appropriate healthcare professional.
