What should you bring to your appointment?

If you visit F&A Medical Center with a new concern, you generally need a referral from your GP. Your GP may send the referral digitally or give you a printed copy to bring.

Please also bring:

• Your health-insurance card
• Your appointment details
• A valid identity document, such as a passport, driving licence, identity card or residence permit
• An up-to-date list of the medicines you use, or the medicines themselves
