Referring to F&A Medical Center
ZorgDomein: You can refer a patient through ZorgDomein. If our center does not appear near the top of your results, search by the Rotterdam location or enter “F&A Medical Center” in the search field.
Email: If you cannot find us in ZorgDomein, you may email the referral to info@fa-mc.nl. We aim to contact the patient on the same or next working day to arrange an appointment.
Paper referral: You may also give the patient a printed referral to bring with them.
