This statement covers personal data processed through fa-mc.nl. It describes only the website and form functions that were verified.

2. Information you provide through the website
The contact and appointment forms ask for your first name, last name, email address and telephone number. Depending on the form, you can also enter a subject and a message. Please do not include more medical information than necessary in a public website form.
An appointment request submitted through the website is not final until F&A Medical Center confirms it.
3. Information recorded automatically
The forms and technical systems may record the source page, IP address, date and time, browser, device and a general country indication. This information is used to process forms, protect the website and manage technical issues.
4. Purposes
Website data is used to:
- answer a contact question or appointment request;
- manage form submissions and make them available to authorised staff;
- limit spam, misuse and technical problems;
- measure website visits and advertising results with the services listed below.
5. Forms and storage
The website uses Fluent Forms for contact and appointment forms and Cloudflare Turnstile to protect forms against automated spam. Form submissions are stored in the WordPress administration. The audited form settings do not contain a separate automatic deletion period. Submissions therefore remain stored until an authorised administrator deletes them.
No marketing or payment integrations were connected to the audited Dutch contact and appointment forms.
6. Analytics, advertising and external services
The technical website review identified the following services:
- Google Tag Manager for managing tags;
- Google Analytics 4 for website usage statistics;
- Google Ads for advertising measurement;
- Microsoft Clarity for website usage analysis;
- Cloudflare Turnstile for form security;
- Google Fonts for fonts;
- Trustindex for displaying reviews.
These external services may receive technical information, including your IP address and browser or device information. Google Analytics, Google Ads and Microsoft Clarity may use cookies or similar technologies. See our Cookie Policy and the privacy information of each supplier for more information.
7. Cookies and consent
Consent is not always required for essential security and technical functions. Prior consent may be required for analytics, advertising measurement and other non-essential tracking technologies. On the date of this review, the website displayed a cookie notice with “Aanvaarden” (Accept) and “Afwijzen” (Reject) choices. You can also block or delete cookies in your browser. 8. Recipients and processing outside the EEA
Website data may be accessible to authorised staff and suppliers supporting the website, forms, email, security, analytics or advertising.
