I have a question about my healthcare invoice. Who should I contact?

Email questions or comments about your invoice to info@fa-mc.nl. We will review your message as soon as possible.

By car:

  1. Take exit 16 Capelle aan den IJssel towards the S109.
  2. Follow signs for Rotterdam-Zevenkamp / Rotterdam-Ommoord.
  3. Continue via President Rooseveltweg and Molenlaan towards Weissenbruchlaan in Hillegersberg-Schiebroek.
  4. Follow Capelseweg, Terbregseweg, Prinses Irenebrug and Molenlaan.
  5. At the roundabout, take the third exit onto Burgemeester F.H. van Kempensingel and continue to Weissenbruchlaan. The center is on your right.

Public transport: From Rotterdam Centraal, take tram 4 towards Molenlaan and get off at Bergse Dorpsstraat. F&A Medical Center is about a one-minute walk away.

Parking: Paid parking is available in front of the center. Free parking may be available farther along Weissenbruchlaan after Sterkkade; always check the current street signs and local rules.

Please bring the following to your appointment:

  • Your health-insurance card or documents
  • Your appointment details
  • A valid identity document, such as a passport, driving licence, identity card or residence permit
  • An up-to-date medication list, or the medication itself

To change or cancel an appointment, contact us by telephone or email as soon as possible. Appointments cannot currently be changed or cancelled online. If you contact us in time, we can offer the time to another patient and arrange a new appointment for you if needed.

Our center focuses on accessible outpatient ENT care. This allows us to organise common assessments and treatments efficiently in a personal clinical setting. The most suitable place for care still depends on your condition and referral.

Our treatment team includes experienced ENT specialists with expertise in different areas of the specialty. We match your concern with an appropriate clinician whenever possible.

Reimbursement depends on your referral, insurer, policy and the type of care. The statutory deductible may apply. Check your policy or contact your insurer before treatment; our team can explain the invoicing process but cannot guarantee a particular reimbursement decision.
