These terms apply to the use of fa-mc.nl. They cover the website only. They do not form a treatment agreement and do not set terms for prices, payment, insurance, cancellation or medical treatment.

The website provides general information about F&A Medical Center, its listed services, contact options and appointment requests. It is not intended as a personal diagnosis or as a replacement for a consultation with a qualified healthcare professional.
Do not use the website for emergencies. Call 112 in a life-threatening situation. For other urgent medical help, contact your general practitioner or out-of-hours medical service.
2. Appointment and contact requests
A form submission is a request for contact or an appointment. An appointment is only final after F&A Medical Center confirms it. Do not include more personal or medical information than necessary in a website form.

You may use the website for normal personal and informational purposes. You must not use it to:

- send harmful code, spam or misleading information;
- attempt unlawful access to the website or stored data;
