Requesting your medical records

If you would like a copy of your medical record, contact us and we will send you a request form. Sign the form and return it according to the instructions. We may ask for identification before releasing medical information.

Once the signed request and any required identification have been received and checked, we will send the records through an appropriate secure channel. Processing time depends on the scope of the request and applicable legal requirements.
